I acknowledge and accept any risks associated with such activities. I hereby release, discharge, and hold harmless North Albemarle Baptist Church, its staff, volunteers, and representatives from any and all liability, claims, demands, or causes of action that may arise from injury, illness, or accident incurred during participation in VBS. In the event of an emergency, I authorize church representatives to seek medical attention for my child/myself and accept financial responsibility for any treatment deemed necessary.
